practitionermap.com is a directory of licensed healthcare practitioners serving the Portland, Oregon metropolitan area. It is an informational resource — not a healthcare provider, not a referral service, and not a source of medical advice. Using this site does not create a patient–provider relationship of any kind.

By using this site you agree to these terms. If you do not agree, do not use the site.

Directory information

Practitioner and clinic information is compiled from publicly available sources including Google Business Profile, state licensing databases, and the federal NPI Registry. We strive for accuracy but do not guarantee that any listing is current, complete, or error-free. Information may not reflect recent changes to a practice’s availability, insurance acceptance, location, or licensure status.

Practitioners may request corrections or removal of their listing by contacting us at the address below. We do not fabricate claims, assign editorial quality ratings, or create reviews.

No reviews, no ratings. practitionermap.com does not host, display, or solicit patient reviews. The verified badge indicates only that a practitioner’s license has been confirmed against the relevant state board database at the date shown — it is not an endorsement.

Claiming a listing

Licensed practitioners may claim their directory listing to add or update information. By claiming a listing you confirm that all information you provide is accurate and that you hold the license you represent. You are responsible for keeping claimed information current.

We may remove or suspend a claimed listing if we determine that information provided is materially inaccurate, that the license cannot be verified against the relevant state board, or that the listing violates these terms.

Featured positions

Featured positions are monthly subscriptions that place one practitioner in the highlighted position on a specific territory (condition × geographic area combination). Featured positions are exclusive — only one practitioner may hold a featured position per territory at any time.

• Billing is monthly. The first month carries an unconditional thirty-day money-back guarantee.
• Cancellation requires thirty days written notice. The position remains active through the end of the paid period.
• We monitor published advertising rules and flag potential compliance issues. This service identifies issues against published rules — it is not legal advice and does not guarantee freedom from board complaints.

Acceptable use

You agree not to:

• Scrape, copy, or systematically extract directory data for commercial use
• Misrepresent your identity or credentials when claiming a listing
• Use this site to present directory content as medical advice or as a personal recommendation
• Attempt to interfere with the security or operation of the site

Limitation of liability

practitionermap.com is provided as-is. To the fullest extent permitted by law, our liability for any claim arising from use of this site is limited to the fees you paid us in the thirty days preceding the claim. We are not liable for decisions made based on directory information, for errors in third-party data sources, or for outcomes of care with any listed practitioner.

Governing law

These terms are governed by the laws of the State of Oregon. Any dispute arising from these terms or your use of the site shall be resolved in the courts of Multnomah County, Oregon.

Changes to these terms

We may update these terms as the service evolves. Material changes will be noted with an updated date at the top of this page. Continued use of the site after a change constitutes acceptance of the revised terms.
